Providing financial assistance to undergraduates and high school seniors who suffer from CHDs, in hopes of reducing some of their burden from an already hard journey with CHD.
APPLICATION CRITERIA
Eligible students are U.S. citizens and either in the process of entering an accredited undergraduate college/university or trade school or are currently enrolled.
A GPA of 3.0 or higher is required as well a transcript from the current academic institution.
Application deadline is February 1st with award notification by April 15th.
Acceptance letter or proof of enrollment for Fall term required before scholarship is awarded but not required for initial application submission.
Applicants must provide a letter from a licensed practitioner stating the applicant has been diagnosed with a CHD. No medical records or progress notes will be accepted.
Applicants should submit a personal statement describing the kinds of obstacles they have had to overcome due to having a CHD and the impact those might have on their educational and life goals for the future.
One letter of recommendation is required.
Scholarship awards are dispersed directly to school in which the applicant is/or will be attending.
The number of scholarships and the award amounts may vary with awards starting at $1,000.
